Wiki-like collaborative development for seamless customer involvement

We have known for a long time that customer/developers constant collaboration is a key element of successful software projects and services. How can we reduce the “impedance mismatch” between even simple requirements such as a user story and the actual application?
Behaviour Driven Development have proven effective thanks to collaboration with users on a textual description of the specifications which is subsequently “completed” by developers.

How can we push the concept even further? how can we create a workflow where the specifications “became” progressively the application? a bit like documentation is progressively sketched and refined in a wiki?
How can we implement these ideas today, with existing Open Source tools? what are the challenges? what are the limitations and issues yet to overcome? real-world experience is included.
